The Clarinet forms management module is designed to allow users to become involved in form design. Using simple office productivity tools, users can work with Certus to develop forms covering any part of the care pathway from case referral and amendment to diagnosis and testing. Forms are available within the system online - these are filled in via the user's browser. They are available offline as MS Word or MS Excel forms, or in PDF.

Axiom Clarinet provides support for both data and process management. Process management is about ensuring the system is up to date with the real world and that each user completes their role in a timely and proper manner. Clarinet provides users with interfaces to complete tasks, such as filling in forms. When one step in a process is complete, the next user should be informed of their task. Clarinet's workflow system can email users when their attention is required, monitors task completion and provide each user with a list of their tasks.

The Clarinet Supervisor groups activities within a case and presents the case coordinator with a timeline showing every important event within the case. The timeline shows active workflows and the activities they contain. Visual representations of activities signal to the coordinator where attention is necessary. A simple click on an activity or workflow provides immediate access to the appropriate part of the system.

Clarinet is an idea platform for clincal records management in genetics. It is capable of operating workflows and recording episodes in line with UK NHS Clinical Genetics Service recommendations for Clinical Genetics as described in the Do Once and Share (DOAS) programme.

Diagnostic work inevitably involves patient samples and testing. In rare disease management such tests are often completed by specialist laboratories. Keeping track of samples and test results can be complex. Axiom Clarinet provides functions to request tests and manage the shipment of samples between labs. Test results are recorded on customised forms and are maintained in the patient record.

Clarinet allows controlled access to case details to expose the key patient information to the professionals involved in the case. The ability to control information access is used within a case referral system to allow opinions to be sought and cases to be discussed online. Case discussion is managed in the Supervisor, where referees can be invited to review cases and to share their views via a discussion. The case coordinator can monitor the discussion process and conclude this with a summary comment or by drawing together views in a summary form.

Clarinet can help manage the complexity of obtaining and recording levels of consent for each patient or family. Configurable consent structures are used to represent the necessary levels of consent for different clinical activities and information storage. Using the consent system a record is constructed of the consent level obtained for a patient over time. Each change to an individual patient or family consent record is easily available in the Supervisor. Scanned records of signed documents can be uploaded to accompany the consent record.

Axiom Clarinet brings all the important patient information together in one place and creates a workspace laid out by you for each patient. Imagine a patient file or dossier full of documents, radiographs and forms. Sort through these items and lay them out on the desk and attach sticky-notes to help remind yourself of the important items. This is the Clarinet Patient Desktop concept, where electronic documents are placed on the desktop for easy reference. Layouts are recorded and restored making Patient Desktop a convenient and efficient access mechanism for patient review.
